Text Box: Cane Creek Pharmacy has expanded our nursing department. We employee four full time nurses. Lori McAdams, RN and Lisa Dee, RN both work in the pharmacy with day to day operations of the pharmacy. Lori is also a licensed North Carolina Adult Care Home Administrator and is very familiar with the rules and regulations of adult and family care homes . Lisa has experience with care plans and Medicare/Medicaid Billing. Our other two nurses are out in our facilities along with our pharmacists to provide our homes with a team approach when providing medication reviews and training.

Our educational in-services and medication reviews are offered to you at no charge. These services are not limited.
Text Box: LTC patients are frequently prescribed antibiotics for respiratory tract infections, skin infections, and UTIs. Problems often arise when these patients are also taking the anticoagulant warfarin.
Antibiotics such as metronidazole, macrolides, quinolones, sulfonamides and the antifungals such as fluconazole can INCREASE the anticoagulant effect of warfarin. Examples such as dicloxacillin, griseofulvin, nafcillin & rifampin can DECREASE warfarin’s effect and increasing clot risks. Fever and inflammation can affect the INR while the antibiotic itself may change Vitamin K production in the gut and alter warfarin’s effect.
An INR should be checked within 3 days after starting any new antibiotic. INR should be monitored frequently during the course of therapy. Continue to monitor after stopping the antibiotic as the body returns to normal. Any of our pharmacists are available to answer your warfarin questions
